SO WHAT IS THE NDIS?
The NDIS empowers people who live with a disability to receive support that is tailored to meet their individual needs and goals. It allows for flexibility and choice in who your service provider/s may be and the services that you receive. You can even control your own funds.
NDIS is a big change with the potential to transform lives.
HOW CAN THE NDIS HELP ME?
The great thing about NDIS is that it’s all about YOU ~ you develop your care plan tailored to your needs and your lifestyle ~ to live a life of your choice!
If you are eligible for the NDIS, you will be invited to develop your personal plan by the NDIA. This meeting will assess the supports and services you need based on your goals and what you want to achieve in the future. From here your individual plan will be created.
Potential services and/or supports that may be funded under the NDIS include health and wellbeing ~ help with household tasks and/or personal care and tasks associated with daily living;social participation ~ assistance to help with participation in community, social economic and daily life activities; independence ~ mobility equipment and/or vehicle modifications; and living arrangements, education, employment and/or therapeutic supports.

HOW DO I APPLY FOR THE NDIS? It's Easy
Step 1
Check your eligibility
Your eligibility for the NDIS will be assessed by the National Disability Insurance Agency (NDIA)
Visit the NDIS website ~ www.ndis.gov.au and follow the link to ‘accessing NDIS’ or phone the NDIS on 1800 800 110.

Step 2
Gather your information
The NDIS will need to know about your disability and how your disability affects your daily life. To help NDIS understand what supports and services you may need you may need to provide evidence ~ i.e reports, tests or letters from you doctor, therapists, family, carers or support staff.
Step 3
Fill out the paperwork
Complete your Access Request Application Form ~ NDIA will direct you on this. If accepted into the NDIS, you will receive a letter from NDIA in about 4 weeks.
Step 4
Developing your NDIS Plan
Once accepted the next step is to develop your individual plan ~ services and supports that meet your individual needs to live the best life that you are able!
