How are services accessed?
There have been significant changes across the aged care sector, with a strong focus on supporting older people to remain living safely and independently at home.
Older Australians can access services through My Aged Care. You can visit www.myagedcare.gov.au or call 1800 200 422 to get started.
If you are unsure where to begin, our team is here to help. We can guide you through the process, answer your questions and support you in getting started with the right services for your needs.

I would like to get out of my house and join a social group ~ what do you offer and what do I have to do to join?
We offer a range of social activities and group programs designed to help you stay connected, active and engaged in your community.
To get started, simply contact our team. We’ll talk through your interests, explain what’s available and help you find the right group for you. We can also guide you through the My Aged Care process if needed.
What kind of qualifications or training do your Support Workers have?
Staff at Liberty are handpicked! We’re a very diverse mob and our clients rate their support workers very highly ~ often referring to them as ‘family’. Please refer to the Our Team page for more information.
How much choice am I allowed to have in the services and/or activiites that I may want?
You have full choice and flexibility in the services and activities you receive.
At Liberty, we take the time to understand what you enjoy, what you need and what matters most to you. Support is tailored to suit your preferences, lifestyle and goals, and can include help at home, getting out into the community, or staying socially connected.
Whether it’s assistance with daily tasks, getting to appointments, or joining social activities and outings, we will work with you to create a plan that is personalised and works for you.
Our team will guide you through your options and adjust your support over time, ensuring it continues to meet your needs.
How do I make a donation to Liberty?
Liberty Community Connect is an award winning, independent, not for profit organization. We are dependent on the good will and generosity of our donors. Liberty is a registered charity and all gifts over $2 are tax deductible.
If you would like to make a donation or leave a bequest please either
- give us ring ~ we would love the opportunity to tell you more about what, why and where we do what we do ~ how your contribution will make a significant difference within your local community
- follow the Donations tab on our website
- automatic bank transfer or
- by cheque.
